Bears, did you go afk? If you are at spawn or in a rank warp afk, a player may have the chance to use an explosive pickaxe within safezone and push you to a possible pvp zone with its knockback.(May be an expliotable bug) This is just a possibility and should this be not the case, then I am not sure on how you got killed.

Use /iteminfo <itemid> to find out the required crafting material, or just do /iteminfo when you are holding the item you desire to find out its id. Its simple.
